Frequently Needed Forms

*All forms must be submitted digitally via the Google form process. Please only fill out the form once when submitting. *

Deadline for all Spring 2026 Registration forms will be Sunday, February 1, 2026 to ensure that the forms can be processed prior to the add/drop deadline. Any forms received after this date are not guaranteed to be processed before the add/drop deadline.

Major/Minor Declaration forms can be submitted starting Monday, October 27, 2025 and ending on Friday, January 30, 2026 at 4:00 PM. The declaration will be effective as of the Spring 2026 semester. The form can be found on SOLAR by going to Student Records and Registration > Enrollment > Undergraduate Major/Minor form.

*To guarantee inclusion in the Department Convocation Program, final submission of the completed Application for Departmental Honors in Biology must be received by bio_registration@stonybrook.edu no later than May 1st.*
