  Medical Leave Semester Withdrawal Request Form - (Available on 1/26/2026 for Spring 2026)

• New Incoming Undergraduates

  New incoming freshmen and transfer students in their first term who decide to withdraw after Day 1 of the semester and before the Add/Drop deadline will need to submit the First Semester Withdrawal Form. This will become available on Monday, January 26th.

  Please see policies outlined within the form pertaining to impacts to your student record.
